Requirement: Navigation Banner for OHIE Tools

Would like to see a navigation banner at the top like OpenMRS Talk - https://talk.openmrs.org so we can point people to other OHIE tools.

@jthomas this is possible. do you want it to appear across all the different OHIE tools or only this one.

@jthomas I have added one with lessons from OpenMRS. Please review and let me know if this is what we want. Then we can figure out how to make it global.

Thank you. We will add the tool bar to the website and wiki when we go live as to not create confusion for people.
