My response to this is that it depends what you mean by “hierarchies”. It’s not generally appropriate to assert subsumption relationships in a valueset. More generally, it’s not a good idea to make statements about meaning in valuesets - meaning should be in the code system.
On the other hand, there’s another use for hierarchy which is navigational assistance - you’re not making any claim about meaning, just presentation. It’s perfectly appropriate for value sets to do this.
The FHIR value set doesn’t provide out of the box support for this because our observation is that this is pretty unusual, but it can be done with extensions
