Exchanging ADT messages between the POC and the SHR

Hi,

We at Regenstrief have come to the conclusion that the SHR would benefit from accepting ADT messages into the SHR.

We’d like to do this to keep track of patient related pieces of information such as admission, discharge, allergies etc.

Whats hot on our list of todo’s are,

A01 - admission to hospital (inpatient)

A04 - Arrival to outpatient clinic

A03 - discharge from either A01 or A04

A08 - update of an ADT message

At the moment, ADT^A08 messages are transferred between the POC and the IOL, but they don’t really make it into the SHR. Therefore, this will involve modifying the IOL to be able to redirect these ADT’s into the SHR.

We wanted to reach out to the community, and hear your thoughts on this. What type of ADT related messages are you interested in exchanging with the SHR ?

Is opening up the IOL to forward ADT’s into the SHR an agreeable solution ?

Hi Suranga,

Within the SHR community and the greater OpenHIE community it has been decided that the use of IHE profiles are likely the best way to go to achieve semantic interoperability between the components of the HIE. I’d rather us focus on moving in this direction than growing the supports for HL7 messages that we will have to profile ourselves. I would recommend that you guys have a look at what IHE profiles could be used to perform the functions that you are anticipating. They may even profile the specific HL7 messages that you mention here. We can then discuss this further as a community.

I think that the top priority for the SHR community should be to explore allowing clinical data to be stored in an SHR via XDS.b using CDA documents for the clinical content. Perhaps, we could discuss the additional use cases that you see as being beneficial on the call later today. Also, we are hoping that Regenstrief will be able to help develop the link between HIEOS and OpenMRS. We can discuss the details of this on the call today as well. I think it would be better to focus efforts on this area for now.
